Required Information
For 911 call and CAD data, fill out the below form. For fire department or medical response reports, contact your local fire department. Click here to find your Fire Agency.
All 911 requests for public records are required to include the following data and be as detailed as possible:
Name
Email
Location of incident
Time and date of incident
The reason why emergency responders were needed
